BackgroundHealth screening is undertaken to identify individuals who are deemed at higher risk of disease for further diagnostic testing so that they may possibly benefit from interventions to modify the natural course of disease. In Singapore, screening tests are widely available in the form of a package, which bundles multiple tests in one session and commonly includes non-recommended tests. There are various ethical issues associated with such testing as they may not be clinically appropriate and can result in more harm (...) than benefit. This article describes the practice of health screening packages, identifies the ethical issues arising from such packages and discusses the implications of these ethical issues on policy and practice of screening in Singapore.MethodsA content analysis of the websites of providers offering general health screening packages to individuals was conducted. A total of 14 health screening package providers were analysed for how packages were conducted and promoted, how clinically appropriate screening tests were, and the price range and composition of screening packages. A normative ethical analysis based on the four principles approach of beneficence, non-maleficence, autonomy and justice in biomedical ethics was used.ResultsTwelve of the 14 providers included non-recommended tests such as tumour markers, treadmill stress tests and MRI scans in their general health screening packages. Package prices ranged from S$26 to S$10,561, with providers charging higher when more tests were included. Health screening packages were broadly conducted in three stages: the offer and selection of a health screening package; medical assessment and performance of screening tests; a post-screening review. While material provided by all providers was factual, there was no information on the potential risks or harms of screening.ConclusionSeveral ethical issues were identified that should be addressed with regard to health screening packages in Singapore. A key issue was the information gap between providers and patients, which may result in patients undergoing inappropriate testing that may be more harmful than beneficial. Health screening packages can stimulate unnecessary demand for healthcare and contribute to an inequitable distribution of healthcare resources. (shrink)

In this essay, I find much to admire and little to disagree with in Shu-Shan L ee ’s use of James Scott’s “public transcript” framework to excavate a theory of political obligation that applies to common people in premodern China. I offer some ways to further explore the implications of Lee’s analysis, in part by connecting Lee’s essay to related work on the obligations of elites. I then build on Lee’s own suggestions of connections to contemporary empirical attitudes and contemporary (...) normative views, asking in what ways Lee’s account of public obligation might be able to make room for the idea that the people, as political children, might one day be able to grow up. In an award-winning essay, Shu-Shan L ee discusses scholarly commentary about obedience to the emperor, focusing on public and hidden records of protest. The thesis of Lee’s essay is that the relationship between authority and subject in imperial Confucianism was built on a conditional obligation of obedience, despite traditional accounts of it as absolute. On his account, the obligation of obedience should be conceived through the rubric of imperial Confucianism as being conditional on fulfillment of reciprocal obligations. As part of (...) my response, I suggest that there is a third way to understand the obligation of obedience through the Confucian lens, which is less transactional than reciprocal obligation, while also providing a plausible alternative to absolute obligation as the correct understanding of imperial Confucian thinking about obedience to the emperor. East Asian countries such as South Korea have recently made headlines for experimenting with different methods to incentivise people to have (more) children, in a bid to reverse declining birth rates. Many such incentives—child benefits, cash bonuses, dating events, and so on—appear morally innocuous at first glance. I will demonstrate in this analysis, however, that they amount to stopgap measures which reveal fundamental shortcomings with the way various nation states are approaching the so-called ‘problem’ of fertility decline.

You ought to believe in accordance with available evidence. This evidential norm, as widely recognized, can be implausibly demanding by requiring you to hold pointless beliefs. In this paper, I first consider some seemingly promising versions of the positive evidential requirement to form beliefs in accordance with your evidence and argue that they either fail to avoid the problem of pointlessness or fail to bind you independently of practical requirements. I then show that even the negative requirement not to believe (...) against your evidence falls prey to the same problem: either it enjoins pointlessness or its binding force depends essentially on practical norms. This is puzzling, as our epistemic practice seems to presuppose evidential standards that are independent of practical rationality. I suggest that the alleged evidential “norms” are evaluative ‘ought’-statements about what it would be epistemically ideal for you to do, which, on their own, do not issue any genuine prescriptions or demands. The growing demand for uterus transplantation (UTx) invites continued philosophical evaluation of the function of UTx (and what constitutes its ‘success’), as well as the recipient eligibility for UTx. Currently, UTx caters to partnered, cisgender women of childbearing age looking to get pregnant and give birth to a biogenetically related child. The medical justification for this—the treatment of uterine infertility—explains the primacy of this practice. However, this dominant conceptualization of UTx does not necessarily capture the diverse needs for which both (...) cis‐ and transgender women might take interest in UTx that are not strictly procreative, such as bodily integrity and gender identity reasons. In this paper, I argue that non‐procreative motivations for uterus acquisition ought to be taken seriously as a matter of non‐discrimination and consistency. (shrink)

This paper argues that in Eudemian Ethics 8.3, virtue’s mean between excess and deficiency is defined by the standard of promoting the most contemplation. Promotion is indirect and constrained by virtue’s other essential features. The chapter’s apparent restriction of the standard to actions concerning natural goods actually serves a dialectical, not a restrictive, purpose. This paper proposes to unify the chapter’s argumentative arc.

A global trend of institutionalised pronatalism situates low fertility as a site of demographic disaster – positioning primarily women’s bodies as both its cause and solution. In light of such demographic dread, assisted reproductive technologies (ART) may be utilized by pronatalist states as a strategy for fertility recovery, rather than as a benefit for individual aspiring parents. In other words, ARTs are at risk of being co-opted by nation-states for problematic demographic designs which do not advance emancipatory goals. The underlying (...) issue of pronatalism, however, is not always explicitly acknowledged in feminist bioethics as a barrier to ethical ART usage. In my paper, I argue that it would be a mistake to assume ART can be used for emancipatory ends if we do not, at the same time, critically investigate its connections to institutionalised pronatalism. Explanation can be distinguished between linguistic practices and metaphysical relations. At least with respect to metaphysical explanation, some are skeptical that any knowledge gained via explanation qua linguistic practices confers knowledge of explanation qua metaphysical relation. I argue that this skepticism is unfounded. Engaging in the linguistic practice of explanation gives us no reason to skeptical in beliefs about corresponding metaphysical relations like causation or grounding. Moreover, those very linguistic practices can provide resources to justify beliefs in those relations. To be conscious—according to a common metaphor—is for the “lights to be on inside.” Is this a good metaphor? I argue that the metaphor elicits useful intuitions while staying neutral on controversial philosophical questions. But I also argue that there are two ways of interpreting the metaphor. Is consciousness the inner light itself? Or is consciousness the illuminated room? Call the first sense subjectivity (where ‘consciousness’ =def what makes an entity feel some way at all), and the second sense phenomenal (...) character (where ‘consciousness’ =def what it feels like to be an entity). I use this distinction—as well as the metaphor of the inner light—to clarify some philosophical questions about whether consciousness comes in degrees, whether consciousness is multidimensional, and whether there are borderline cases of consciousness. This paper defends virtual veridicalism, according to which many perceptual experiences in virtual reality are veridical. My argument centers on perceptual variation, the phenomenon in which perceptual experience appears all the same while being reliably generated by different properties under different circumstances. It consists of three stages. The first stage argues that perceptual variation can occur in color perception without involving misperception. The second stage extends the argument to perceptual variation of space, arguing that it is possible for individuals to (...) perceive distinct physical spaces as having the same experiential space without suffering from systematic misperception. The final stage proceeds to argue that perceptual variation without misperception in color and spatial perception can occur across virtual and ordinary environments. In that sense, given that ordinary experiences are presumably veridical, experiences in virtual reality are also veridical. (shrink)

Scientific theories often allow multiple formulations, e.g., classical mechanics allows Lagrangian and Hamiltonian formulations. While we count them as equally true, it has been suggested that one formulation can still be more metaphysically perspicuous than another. This paper provides a new account of metaphysical perspicuity, offering both descriptive and revisionary components: As a descriptive component, we examine how metaphysical perspicuity has been conceptualized in the literature. As a revisionary component, we challenge the conventional conception that associates metaphysical perspicuity with other (...) neighboring notions. This paper defends a theory of how it is that games are identical both at a time and over time. I argue that this view is an improvement over the account that Michael Ridge defends in his paper “Individuating Games”. This improvement involves a distinction between two kinds of individuation: constitutive individuation and relational individuation. I argue that this distinction does a better job at solving the various puzzles that are related to the synchronic and diachronic identity of games. Bio-heteronormative conceptions of the family have long reinforced a nuclear ideal of the family as a heterosexual marriage, with children who are the genetic progeny of that union. This ideal, however, has also long been resisted in light of recent social developments, exhibited through the increased incidence and acceptance of step-families, donor-conceived families, and so forth. Although to this end some might claim that the bio-heteronormative ideal is not necessary for a social unit to count as a family, a more (...) systematic conceptualization of the family—the kind of family that matters morally—is relatively underexplored in the philosophical literature. This paper makes a start at developing and defending an account of the family that is normatively attractive and in line with the growing prevalence of non-conventional families and methods of family-formation. Our account, which we call a constitutive-affirmative model of the family, takes the family to be constituted by an ongoing process of relevant affective and affirmative relations between the putative family members. This paper clarifies the phenomenon Edmund Husserl referred to as “solitary speech,” defending his account against the deconstructionist claim that it serves as a mere prop for his transcendental project. To do this, first, I examine how this idea has been misunderstood by Jacques Derrida in Speech and Phenomena. Second, I present Husserl's account of solitary speech as a strictly phenomenological-psychological analysis of a concrete unit of psychic experience, drawing on Husserl's broader view on the relationship between psychology and phenomenology. Third, I compare Husserl's account to Lev Vygotsky's empirical psychological account of “inner speech” to demonstrate their affinity, and to re-establish the misunderstood theme of solitary speech as a concrete linguistic and cognitive phenomenon. This not only corrects the deconstructionist bias but sets a proper approach to the phenomenon of solitary speech, which can contribute to a broader account of the linguistic and cultural formation of human consciousness. This article illuminates the open ending of Christopher Nolan's film Inception (2010) in light of Friedrich Nietzsche's philosophy. Drawing particularly on Nietzsche's notions of ascetic ideals and honest illusions, the article contends that Cobb's refusal to look at the spinning top can be seen not necessarily as his renunciation of autonomy but as his new attempt to affirm his existence and create meanings. Mal's tragic death has turned Cobb into an ascetic idealist who paradoxically resorts to self-torture to alleviate his (...) pain and suffering. Only when he sees the destructive power of his sense of guilt about Mal does he come to realize that what he really needs is not exactly the ability to distinguish the dream world from reality per se but the ability to make things seem and feel real – that is, in Nietzsche's words, honest illusions. Faced with the challenge to regain the capability to see and appreciate illusions as illusions without deceiving himself into believing that they are real, Cobb takes “a leap of faith” with Saito into the world to which he wishes to belong and finally embraces the moment of seeing his children's faces, which, fortunately for him, seem real – that is, “good” – enough. This letter responds to the article “Neither Ethical nor Prudent: Why Not to Choose Normothermic Regional Perfusion,” by Adam Omelianchuk et al., in the July‐August 2024 issue of the Hastings Center Report.

The harmony thesis about a virtuous person, widely held by neo-Aristotelians, supposes that someone highly vulnerable to temptation is not virtuous at all. However, is that the only plausible picture of a virtuous person’s psychology? This essay aims to offer an alternative picture by discussing the account of virtue in the thought of Mengzi 孟子 and his conception of moral exemplars. First, I analyze the Mengzian moral exemplar as depicted in _Mengzi_ 1A7—specifically, the susceptibility of the nobleman (_junzi_ 君子) to (...) compassion and the relevant temptation. Then, I explain how this differs from neo-Aristotelian virtue ethical theories. This passage shows that there is a certain price to being virtuous in Mengzi’s virtue theory. By examining various ways of addressing this challenge, I explore Mengzi’s solution to it, which will shed light on Mengzi’s own way of specifying virtue and its significant advantage as a potential alternative to certain other approaches. But in his newest book, philosopher Lee McIntyre shows how we've now reached a watershed moment for ignorance in the modern era, due to the volume of misinformation, the speed with which it can be digitally disseminated, and the savvy exploitation of our cognitive weaknesses by those who wish to advance their ideological agendas. In _Respecting Truth: Willful Ignorance in the Internet Age_, McIntyre issues a (...) call to fight back against this slide into the witless abyss. In the tradition of Galileo, the author champions the importance of using tested scientific methods for arriving at true beliefs, and shows how our future survival is dependent on a more widespread, reasonable world. On Kant's view, we are rational beings who are morally responsible for our actions. The main goal of this paper is to show that this Kantian view of ourselves is not undermined by the Manipulation Argument, which is currently the biggest challenge to compatibilism. To this end, I argue that a Kantian account of freedom offers a new soft-line reply to this argument. On this Kantian account, moral responsibility requires not only positive freedom but also negative freedom. An agent is (...) free in the positive sense just in case the agent has the rational capacity to act in compliance with moral norms. And an agent lacks negative freedom regarding their actions when these actions are determined by internal or external factors beyond their rational control. Based on this Kantian account, I provide a principled explanation of why there is a significant difference between manipulated actions and merely causally determined actions regarding moral responsibility. In addition, I bring out the distinctive characteristics of my Kantian account further by comparing it with two competing conceptions of freedom: leeway freedom and sourcehood freedom. (shrink)

There are a wide variety of potential applications of artificial intelligence (AI) in Defence settings, ranging from the use of autonomous drones to logistical support. However, limited research exists exploring how the public view these, especially in view of the value of public attitudes for influencing policy-making. An accurate understanding of the public’s perceptions is essential for crafting informed policy, developing responsible governance, and building responsive assurance relating to the development and use of AI in military settings. This study is (...) the first to explore public perceptions of and attitudes towards AI in Defence. A series of four focus groups were conducted with 20 members of the UK public, aged between 18 and 70, to explore their perceptions and attitudes towards AI use in general contexts and, more specifically, applications of AI in Defence settings. Thematic analysis revealed four themes and eleven sub-themes, spanning the role of humans in the system, the ethics of AI use in Defence, trust in AI versus trust in the organisation, and gathering information about AI in Defence. Participants demonstrated a variety of misconceptions about the applications of AI in Defence, with many assuming that a variety of different technologies involving AI are already being used. This highlighted a confluence between information from reputable sources combined with narratives from the mass media and conspiracy theories. The study demonstrates gaps in knowledge and misunderstandings that need to be addressed, and offers practical insights for keeping the public reliably, accurately, and adequately informed about the capabilities, limitations, benefits, and risks of AI in Defence. This research proposes that broad-based employee share ownership (ESO) affects corporate environmental performance (CEP). Drawing upon corporate governance literature, social exchange theory, and stakeholder utility theory, we propose that employees as owners adopt more favorable attitudes toward beneficial outcomes for CEP, and that the broad-based impact of ESO overwhelms the impact of CEO ownership. Also, we propose that these relationships are contingent upon trade union presence as a form of worker voice that amplifies the ESO influence on CEP. The empirical (...) results from U.S. publicly traded companies from 2009 to 2016 indicate that broad-based ESO is linked to positive CEP by companies, and provides some support for all proposed hypotheses. This study provides scholarly and practical implications for the effects of shared capitalist modes of ownership and employee engagement on corporate sustainability. (shrink)

Background Nurses providing care to patients with end-of-life or terminal illnesses often encounter ethically challenging situations leading to moral distress. However, existing quantitative studies have examined moral distress using instruments that address general clinical situations rather than those specific to end-of-life care. Furthermore, qualitative studies have often been limited to participants from a single unit or those experiencing moral distress-induced circumstances. A comprehensive and integrated understanding of the overarching process of moral distress is vital to discern the unique circumstances surrounding (...) end-of-life care and its consequential impacts. Research objectives To explore the moral distress experiences of nurses who are frequently involved in caring for patients with end-of-life or terminal illnesses and apply it to two existing theories: the model of moral distress and the ecological model. Research design A qualitative descriptive approach was employed. Participants and research context Seven focus group interviews involving 30 nurses were performed. The subsequent transcriptions underwent rigorous content analysis. Ethical considerations We obtained Institutional Review Board approval from a university. Focus group interviews were conducted with nurses who agreed to participate and signed the consent form. Findings The moral distress-inducing factors and nurses’ perceived impact of moral distress were identified and categorized based on moral distress theories and ecological models. A total of 15 categories and 30 subcategories across the following 4 domains were derived: (1) intrapersonal, (2) interpersonal, (3) organizational, and (4) structural factors. Conclusions End-of-life-specific circumstances induced moral distress among nurses, with both negative and positive impacts identified. Effective organizational and policy support is essential to manage conflicts, form a healthy organizational culture, provide training, and prevent unnecessary expenses due to the negative consequences of moral distress. Just war theory has long held that the aim of a just war is peace, and not victory. Peace, however, does not feature in either of the two traditional pillars of just war theory: jus ad bellum (which governs the conditions under which we may go to war) and jus in bello (which governs the scope and manner of killing in war). This paper examines the question, which has so far been ignored in the literature, of how exactly just war (...) theory orients a war towards peace. Establishing this foundational claim, which I will refer to as the Peace Claim, is crucial in order for just war theory to hold the middle ground between its two main rivals, realism (which holds that we must pragmatically pursue victory) and pacifism (which holds that we must nonviolently pursue peace). In Broken Hegemonies, Reiner Schürmann looks to Meister Eckhart as the preeminent thinker of the “destitution of natura” as a hegemonic fantasm. He indicates that, to some extent, Ockham is also a thinker of the destitution of this fantasm. The problem, according to Schürmann, is that Ockham reverts to a natural law morality in his political writings. This essay investigates whether Schürmann is correct in this assessment, shows that he is not entirely correct, and then points to the ways in (...) which thinking through this issue points out some interesting features of Schürmann’s overall project. In this paper I argue that the ontological interpretation of the concepts of reduction and emergence is often misleading in the philosophy of science and should nearly always be eschewed in favor of an epistemological interpretation. As a paradigm case, an example is drawn from the philosophy of chemistry to illustrate the drawbacks of “ontological reduction” and “ontological emergence,” and the virtues of an epistemological interpretation of these concepts.

Biobanks are essential biological database resources for the scientific community, enabling research on the molecular, cellular, and genetic basis of human disease. They are crucial for computational, data‐driven biomedical research, which advances precision medicine and the development of targeted therapies. However, biobanks often lack racial and ethnic diversity, with many data sets predominantly comprising individuals of white, primarily northern European, ancestry. Establishing or enhancing biobanks for the inclusion of historically underrepresented populations requires meticulous ethical and social planning beyond logistical, legal, (...) and economic considerations. This guide provides a roadmap for building and sustaining diverse biobanks, emphasizing ethical guidelines and cultural sensitivity. We highlight the importance of obtaining informed consent from donors, respecting their bodily autonomy, and the economic and research benefits of diverse biobanks to enable precision medicine, drug discovery, and industry‐academic partnerships. Prioritizing key ethical and social considerations allows biobanks to advance scientific knowledge while upholding the rights and autonomy of underrepresented populations. Diversity in biobank sample collection enhances research outcomes by ensuring findings are representative and applicable to various human population groups, fostering trust, promoting inclusivity, and addressing health disparities while informing health policy. This is vital to ensuring biobanking efforts contribute meaningfully to the advancement of health equity. (shrink)

A famous mathematical theorem says that the sum of an infinite series of numbers can depend on the order in which those numbers occur. Suppose we interpret the numbers in such a series as representing instances of some physical quantity, such as the weights of a collection of items. The mathematics seems to lead to the result that the weight of a collection of items can depend on the order in which those items are weighed. But that is very hard (...) to believe. A puzzle then arises: How do we interpret the metaphysical significance of this mathematical theorem? I first argue that prior solutions to the puzzle lead to implausible consequences. Then I develop my own solution, where the basic idea is that the weight of a collection of items is equal to the limit of the weights of its finite subcollections contained within ever-expanding regions of space. I show how my solution is intuitively plausible and philosophically motivated, how it reveals an underexplored line of metaphysical inquiry about quantities and locations, and how it elucidates some classic puzzles concerning supertasks. This combination of views undermines the interpretations of both the argument’s main critics and its main defenders. A more adequate interpretation, drawing on the idea of inapt hedonic experiences which fail to reflect the pleasantness of their objects, points towards a reassessment of the Argument’s place in the sequence ending Bk. This article examines the ways in which the language of legal rights is invoked by those seeking to improve the treatment of animals. Drawing from a range of analytical, realist, and critical legal and social theorists, it argues that certain argumentative techniques commonly employed to justify the extension of legal rights to animals may serve to strengthen and reproduce the very forms of exploitation they seek to challenge. The article begins by identifying and critiquing the binary characterisation of rights/welfare and (...) property/personality in liberal animal law scholarship. It then employs the insights of Theodor Adorno and Walter Benjamin to expose and critique various appeals to an ‘exterior’ or ‘extra-legal’ domain which functions to stabilise the meaning of these doctrinal categories. In doing so, it explores the strategic viability of rights discourse in the animal advocacy movement with a view to highlighting the limitations of liberal constructions of animal rights. (shrink)

Ugar and Malele write that generic machine learning (ML) technologies for mental health diagnosis would be challenging to implement in sub-Saharan Africa due to cultural specificities in how those conditions are diagnosed. For example, they say that in South Africa, the appearance of ‘schizophrenia’ might be understood as a type of spiritual possession, rather than a mental disorder caused by a brain dysfunction. Hence, a generic ML system is likely to ‘misdiagnose’ persons whose symptomatology matches that of schizophrenia in the (...) South African context. The authors thus claim that ‘a generic or universal design cannot be effective given the heterogeneity of value judgements in defining what mental health disorders are in different contexts’.1 Should we take this to mean that ML systems ‘should not be designed with a generalised perception of mental disorders,’1 as the authors suggest? On the contrary, my view is that generic ML can be useful, with the caveat that issues of cultural sensitivity may engender translational challenges in various national and cultural contexts. Yet, if it can be demonstrated that a generic ML system can reliably and accurately pick up on shared symptomatologies across cultures despite what any individual or community might believe about their actual causes, this aspect is surely the function and value of diagnostic ML which we should take to matter. Cultural gaps in aetiological understandings of mental health conditions make it all the more important to advance generic ML as a calibrating diagnostic tool. Nuclear waste is ethically contentious, concerning institutional trust, community engagement and the role nuclear plays in different sociotechnical configurations of energy futures. Using Q-methodology with a diverse UK-based stakeholder group, we find three emergent discourses: a) ‘Managing a distrustful public’, b) ‘Fair and democratic nuclear waste decision-making’, and c) ‘Putting the experts in control’. Although multi-stakeholder support is expressed for geological disposal of wastes, disagreements arise toward the ethics of nuclear-powered energy futures and for community decision-making roles. We recommend that (...) policy authorities must first strengthen community withdrawal rights, and second, renew deliberative democratic decision-making mechanisms within energy policy strategy. Jean Bodin’s analysis in Six Livres de la République is often understood as evidence of his alleged political absolutism. This article examines Bodin’s theory of offices to argue that this is a misguided view of Bodin’s political thought. I begin by revisiting Bodin’s distinction between the “sovereignty” and the “government” of the state. It is in the analysis of the latter that Bodin constructs a normative doctrine warning of the dangers of “ seigneurial” rule. As I show, Bodin’s purpose was (...) to reject seigneurial rule by contrasting it with lawful rule, the mode of government in which public power was discharged according to law and custom, not by seigneurial will. Essential to Bodin’s analysis was a concept of public office that envisioned the officer as an independent intermediary “borrowing” public powers, not from higher magistrates or from the prince (as medieval lawyers traditionally argued), but only from the impersonal state.
